Annual Health and Medical Record: This form records basic health information about a Scout and all unit members who participate in outdoor activities. Parents complete parts A and B of the form. Part C of the form requires an examination by a physician and must be signed by the Scout's doctor. This form must be on file with the troop before the Scout can participate in a campout or other outdoor activity that lasts more than 72 hours or is strenuous in nature. The form is valid for twelve months from the date of the parent's signature or the date of the physician's signature whichever is earlier. Troop 467 Annual Medical /Trip Release: This form provides emergency contact and insurance information for a Scout and serves as a permission slip and release form. Parents must fill out a new form each year. A Scout may not participate in an outing until a parent completes this form and it is on file with the troop. Separate release forms are required for summer camp and high adventure trips. Demographics Form: Annually, and as parents' information changes, we ask parents to update their personal demographic profile for the troop's records. Youth Application: Parents of new Scouts must submit a youth application to register the Scout with the Boy Scouts of America. This must be done only once. Each year, after the initial registration, the troop automatically re-registers Scouts with the National Council. |
