Annual Registration Forms

FormsAt the start of each Scouting year, parents receive a packet of information and forms to fill out. Parents are asked to complete the forms and return them to the troop before the first campout of the year. The troop needs these forms to operate and to comply with the rules of the Boy Scouts of America.

All forms are available on the troop's forms page.

Annual Health and Medical Record:  This form records basic health information about a Scout and all unit members.  Parents complete part A and part C of the form.  Part B of the form requires an examination by a physician and must be signed by the Scout's doctor.   This form is required for all Scouts and unit members.  This form must be on file with the troop before the Scout can participate in a campout or other outdoor activity. The form is valid for 12 calendar months from the date of the parent's signature, or the date of the physician's signature whichever is earlier. 

Troop 467 Medical and Trip Release:   This form provides emergency contact and insurance information for a Scout and serves as a permission slip and release form.  Parents must fill out a new form each year. A Scout may not participate in an outing until a parent completes this form and it is on file with the troop.  Separate release forms are required for summer camp and high adventure trips.

Demographics Form:  Annually, and as parents' information changes, we ask parents to update their personal demographic profile for the troop's records.

Youth Application:  Parents of new Scouts must submit a youth application to register the Scout with the Boy Scouts of America. This must be done only once.  Each year, after the initial registration, the troop automatically re-registers Scouts with the National Council.
